We have there an identity by semantic similarity and an identity by spatial contiguity, pear-heart, while the landscape that is seen across the heart  is only filler.  

This bond is finally  fixed by the first line of the writing under the pear: "Amante della pera / Lowers of pears . . .   with a semantic erotic-gastronomic identity played on the polysemy  of "amante "/ a lover as an erotic partner, or "amante" = who likes . . .  pears, now.
